At Med Plus After Hours Clinic, PC, we have policies in place to protect you and best serve your needs. Please get familiar with our policies before your visit.

• Present your insurance card
• Present a picture ID prior to services rendered
• Complete registration forms
• Update registration forms (if not dated in the current fiscal year)
• Update forms if you have moved, changed your phone, or have insurance changes (notify the receptionist upon arrival)
• Provide copays at the time of service

You must be at least 18 years of age to get treatment without a parent or guardian present. The only exceptions are mandated by Michigan state laws.
